Qualifications

• Master's degree in a human services field and two (2) years of relevant experience.
• Preference will be given to applicants with experience in behavioral health, case management, or community-based services.
• Applicants who demonstrate a strong interest in serving older adults and the ability to work effectively with complex behavioral health needs are encouraged to apply.

Responsibilities

• Facilitates consumer access to a variety of mental and physical health treatment providers, including scheduling doctors’ appointments and accompanying consumers.
• Provides supportive counseling to older consumers on life adjustments, stress management, psycho-education and coping strategies.
• Provides intensive mental health case management to older adults with serious mental illness (SMI).
• Monitors consumer health status and adherence to medications and treatment.
• Coordinates with other service providers to ensure needs are met and resources are accessed.
• Links clients to community resources and helps with entitlements and benefits.
