Job summary

Acclaim Health, the clinical practice group affiliated with JPS Health Network in Fort Worth, seeks a Physician in Geriatric Medicine for outpatient care. You will manage a panel of older patients across primary care and geriatrics with a focus on high-quality, compassionate care.

You will collaborate with a large team of physicians and advanced practice providers, participate in education and research, and contribute to community health initiatives within the Acclaim/JPS network.

Qualifications

• Residency in Internal Medicine or Family Medicine with a dedicated Geriatric Medicine fellowship.
• Board certification by ABFM or ABIM; CAQ in Geriatric Medicine within 5 years considered.

Responsibilities

• Coordinate multidisciplinary care for your patient panel.
• Review medical records, diagnose, treat, and provide care management and education.
• Make appropriate patient referrals and ensure continuity of care.
• Support empathy, dignity, and respect for aging populations.
• Monitor guidelines from JPS Health Network and Acclaim Health for patient care.
• Provide evidence-based, patient-centered care to improve outcomes and satisfaction.
• Collaborate with physicians, residents, and staff to deliver quality care.
• Support academic, educational, and research endeavors.
• Assess and manage chronic and acute conditions in patients with multiple comorbidities.

Education

Internal Medicine or Family Medicine residency and Geriatric Medicine fellowship (ACGME)

ABFM or ABIM board certification; CAQ consideration within 5 years
