Job summary
Hoag Health is seeking a Geriatric Medicine Physician to join the Center for Brain Health in Newport Beach, CA. The role emphasizes longitudinal geriatric care, polypharmacy review, deprescribing, and advanced care planning within a collaborative care model.
The candidate should be trained in Internal Medicine or Family Medicine, board-certified, with CA licensure or eligibility, and a commitment to multidisciplinary teamwork across behavioral neurology, geriatric psychiatry, and palliative
Qualifications
- MD or DO degree from an accredited medical school.
- Residency in Internal Medicine or Family Medicine.
- Board Certified in Internal Medicine or Family Medicine.
- Fellowship in Geriatric Medicine preferred.
- Active California medical license or ability to obtain one.
- Active CA DEA license or ability to obtain one.
Responsibilities
- Provide longitudinal follow-up and comprehensive medical support for geriatric patients.
- Direct polypharmacy review and deprescribing initiatives.
- Lead advanced care planning and manage complex comorbidities across encounters.
- Oversee and coordinate care within the shared care plan with multiple specialists.
- Navigate credentialing process with Hoag Health and maintain clinical quality.
