Job summary

Strive Health in Wisconsin seeks a Licensed Professional Counselor (LPC) to provide psychotherapy and psychosocial support to residents in skilled nursing facilities. The LPC conducts assessments, delivers individual and group therapy, and collaborates with the interdisciplinary team to ensure regulatory compliance.

You will document sessions for Medicare and facility requirements, participate in care plan meetings, and educate facility staff on psychosocial needs and referral processes.

Qualifications

• Master’s in counseling or related field; CACREP preferred but not required.
• Enrollment as Medicare Part B provider or eligible.
• Experience with geriatric populations or long-term care preferred.
• Strong interpersonal, documentation, and communication skills.
• Ability to work independently within a collaborative, team-based environment.

Responsibilities

• Conduct initial assessments and develop individualized treatment plans.
• Provide psychotherapy services to residents.
• Deliver individual, group, and family counseling for depression, anxiety, grief, and behavioral health.
• Document sessions per Medicare and facility requirements.
• Participate in care plan meetings with the interdisciplinary team.
• Collaborate with Strive psychiatric nurse practitioners and facility staff for coordinated care.
• Educate facility staff on recognizing psychosocial needs and referral processes.
