Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities across New Castle, PA. Clinicians can build their own schedules and may incorporate telehealth to extend access for patients in the Geriatric House Calls model.

Responsibilities include evaluations, customized care plans, and delivering evidence-based therapy to improve mobility and independence.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and aging in place.
