Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one, in-home care for older adults in Nanuet, NY and surrounding communities. You’ll treat patients in their homes or senior living settings, using our Geriatric House Calls™ model to improve mobility, independence, and quality of life.

Qualifications

• Valid Physical Therapy license in the state(s) of practice
• Degree from an accredited Physical Therapy program
• Current CPR certification
• Basic computer and EMR proficiency

Responsibilities

• Provide one-on-one physical therapy to older adults in-home and in senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support wellness, mobility, independence
• Document effectively within FOX’s EMR system
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote aging in place
• Provide telehealth / virtual care when appropriate
