Job summary

CIMPAR,S.C. is seeking a Family Nurse Practitioner to provide high-quality geriatric care in skilled nursing facilities. The role includes admissions, follow‑ups, discharge planning, and ongoing management of rehab and long‑term care patients.

You will work with supervising physicians, facility leadership, and care teams to support positive outcomes. Illinois APRN license and FNP certification are required. DEA and IL Controlled Substances licenses preferred.

Qualifications

• Current, unrestricted APRN license in Illinois.
• National FNP certification.
• DEA and IL Controlled Substances licenses.
• Experience with adult/geriatric/p Skilled nursing and post‑acute care preferred.
• Strong clinical judgment, communication, organization, and time management.

Responsibilities

• Provide full‑time clinical coverage by rounding at assigned facilities.
• Evaluate and treat rehab and long‑term care patients.
• Complete assessments, document findings, and develop treatment plans.
• Order, review, and interpret diagnostic tests; prescribe treatments.
• Collaborate with physicians and care teams to coordinate patient care.
• Document care plans, billing, and regulatory requirements.
