Job summary

Avalon is seeking a Registered Dietitian to join our facility in New Jersey. The role focuses on clinical nutrition, developing individualized care plans, and ensuring nutrition services align with medical goals for residents in skilled nursing settings.

Qualifications

• Bachelor’s degree in Dietetics, Nutrition, or related field.
• Current Registered Dietitian (RD) credential.
• Minimum of 2 years of clinical experience preferred.
• Strong expertise in clinical nutrition, preferably within geriatric, rehabilitation, or skilled nursing settings.
• Knowledge of applicable federal, state, and local regulations governing healthcare and nutritional services in skilled nursing environments.

Responsibilities

• Assist in developing and implementing food service procedures.
• Review and develop corrective action plans for deficiencies identified during surveys.
• Maintain and monitor an ongoing quality assurance program for Food Services.
• Review residents’ dietary requirements per physician-ordered diet plans.
• Develop individualized nutrition care plans, including regular and specialized diets.
• Support the Discharge Planning Coordinator with nutrition components of discharge plans.
• Monitor residents’ nutritional status and adjust diet plans as needed.
• Collaborate with interdisciplinary teams to align nutrition care with treatment goals.
