Job summary

InnovAge Holding Corp. is seeking a Clinic RN to provide skilled nursing care to participants at the Florida clinic. You will assist in examinations, implement orders, triage calls, schedule outside appointments, and administer medications while educating participants on self-management.

The role requires an active RN license in Florida, BLS/First Aid, and at least 1 year caring for the frail or elderly. Preferred: a nursing degree and bilingual abilities to serve a diverse participant base.

Qualifications

• 1+ year working with frail or elderly individuals.
• Proven ability to work in a multidisciplinary team.
• Ability to educate patients and families on care plans.

Responsibilities

• Provide skilled nursing care per InnovAge policies and regulations.
• Implement provider orders and coordinate care for participants.
• Triages calls and schedules outside appointments as needed.
• Administer medications and educate self-administration to participants.
• Document medical information and maintain records.
