Job summary

NHC HealthCare Charleston is seeking a Case Manager/Social Worker to coordinate social services for geriatric patients, leveraging expertise in the case management process and funding resources.

The role requires collaboration with the Director of Social Work Services, maintaining patient confidentiality, and adherence to center policies while supporting the mission of the Better Way culture.

Qualifications

• Bachelor’s or Master’s degree in social work from an accredited program.
• Knowledge of the case management process and geriatric patient needs.
• Understanding of Medicare Part A, B, C and D, Medicaid and managed care funding.
• Ability to communicate effectively, both verbally and in writing.

Responsibilities

• Maintain the quality of social services rendered by the center.
• Report to the Director of Social Work Services the current status of social services and needs.
• Interpret center policies to concerned parties.
• Consult with the Director and regional social work consultant on issues needing policy attention.
• Maintain confidentiality of professional information.
• Plan time off with the Director of Social Work Services.
• Attend annual NHC-sponsored social work conferences as required.
