Qualifications

• Active Ohio LSW license required.

Responsibilities

• Assess residents’ psychosocial, emotional, and mental health needs.
• Complete admissions, care plans, and discharge planning documentation.
• Collaborate with nursing, therapy, and leadership teams to support resident outcomes.
• Advocate for resident rights, dignity, and individualized care planning.
• Provide counseling and support to residents and families.
• Coordinate and maintain community resources and services.
• Ensure timely, accurate documentation in compliance with regulatory standards.
• Support implementation of policies related to social services and resident care.
