• Experience: 1 year

Responsibilities

• Visit designated facilities with supervisor per schedule
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions
• Document visit notes and maintain detailed records of interactions and interventions
• Collaborate with psychiatric professionals to ensure comprehensive care, including monthly meeting with psychiatric NP
