Job summary

Reid Health is seeking an RN for the PACE Home Care program in Pace, FL. The role focuses on developing and implementing home care services for PACE participants, coordinating DME and nursing home care, and ensuring quality service to meet client needs.

The position requires 1 year of RN experience in geriatric services, ASN (BSN preferred), and licensure in Indiana. Certification in BLS/ACLS/CPR is expected, with a Day shift, 40 hours weekly.

Qualifications

• ASN required.
• BSN preferred.
• 1 year RN experience in geriatric services.
• 3+ years supervision in geriatric/community health setting desired.

Responsibilities

• Assess home care needs of frail elderly population using nursing process.
• Participate in interdisciplinary team to develop and revise care plans.
• Coordinate home care services including nursing, PT, OT, SLP, and personal care.
• Coordinate and authorize Durable Medical Equipment.
• Supervise nursing care in nursing homes per program standards.
• Engage in Home Safety Assessments and IDT meetings.
