Job summary

Alder is seeking an Aging Life Care Professional to assess, plan, and coordinate care for older adults in the Northeast. This field-based, client-facing role requires travel to appointments and a hybrid option for work from the Mamaroneck, NY office or remote when not with clients.

You will conduct comprehensive assessments, create goal-driven care plans, coordinate care across doctors and services, and advocate for clients while navigating family dynamics.

Qualifications

• Advanced degree in social work, nursing, geriatrics or related field is preferred and licensure may be required.
• Experience with older adults and families; strong clinical and assessment skills.
• Excellent interpersonal and communication skills; ability to work with diverse families.

Responsibilities

• Conduct comprehensive assessments of clients’ functional, cognitive, medical, social, and emotional needs.
• Assist with aging decisions, long-term care planning, and service coordination.
• Create goal-oriented care plans and update them as needed.
• Coordinate care across doctors, services, and referrals.
• Advocate for clients and facilitate family discussions and decision-making.
