Greater Lynn Senior Services, Inc. seeks a Geriatric Support Services Coordinator (GSSC) to join a SCO care team, coordinating community-based services for seniors in collaboration with SCO nurses and program managers.

The role emphasizes needs assessment, inter-disciplinary coordination, and detailed recordkeeping in a compliant framework. The position requires a BA/BS with elder care experience, strong computer and communication skills, and the ability to manage multiple tasks while engaging

Qualifications

• Bachelor's Degree with two years of elder case management
• Proficiency in Word, Excel, PowerPoint
• Excellent communication skills including writing and public speaking
• Customer service and recordkeeping experience
• Strong organizational and time management skills
• Ability to work with diverse backgrounds

Responsibilities

• Conduct comprehensive needs assessments as part of an interdisciplinary team
• Coordinate and monitor consumer-centered community-based services
• Interface with formal and informal support systems
• Maintain records in compliance with regulations
• Coordinate with SCO Nurse Case Managers and SCO Program Manager
• Do not provide GSSC services for other SCO contractors
