Job summary

UP Health System - Portage seeks an Elder Associate to provide compassionate, hands-on support to older adult patients in our community hospital. In this role, you’ll assist with daily living activities, mobility, safety checks, and comfort measures while closely following care plans developed by nurses and providers.

You will monitor and report changes in condition, maintain accurate documentation, and support a clean, safe environment.

Qualifications
