Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full-time GIP hospice Registered Nurse to work Monday - Friday from 8 AM to 4 PM in the Morris, Essex, Union, and Somerset County area, including weekends and on-call as required by department. In this position you will work with adult and geriatric patients of all backgrounds who require hospice care at one of our hospital sites, sometimes in their homes as needed. You are compassionate, thoughtful, and comfortable working with elderly and terminally ill patients. You will be responsible for managing a heavy caseload (up to 4 patients a day); time management skills are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs.
Entry level offers start at $43.00 / hour and increase based upon experience. Mileage reimbursement ($.70) included. Internal referral bonus: $5,000
Reports to: Assistant Manager
Minimum of one-year experience in acute, subacute or home/hospice care setting. Experience using an EMR, excellent critical thinking and communication skills.
Performs patient assessments.
Ensures appropriate utilization of services
Completes discharge assessments
Uses age appropriate teaching methods for patients and families
Assumes responsibility for Case Management
Maintains safe environment for patients and co-workers
Attends monthly staff meeting
Assumes responsibility for case management
Participates in Performance Improvement Program
Assists with growing agency business
