Job summary

Northshore Health Centers seeks a licensed Dentist to provide comprehensive dental care within the PCMH model. You will diagnose, treat, and educate patients while adhering to clinic protocols and HIPAA/OSHA requirements.

The role requires Indiana licensure (CSR) and a DDS/DMD from an accredited program, with a DEA license. Experience in healthcare settings is preferred, and on-site care is expected in Hammond, IN.

Qualifications

• Licensed to practice dentistry in Indiana.
• CSR license in Indiana.
• DEA license.
• Graduate of an accredited dentistry program.
• Two years of healthcare experience.

Responsibilities

• Performs quality clinical and radiographic evaluations to diagnose and treat diseases of the oral cavity
• Promotes oral health by providing educational information on all phases of preventative and restorative dentistry and oral hygiene
• Reviews and follows dental clinic protocols, policies, and PCMH procedures
• Documents and follows treatment plans with informed consent prior to rendering treatment
• Manages medical/dental history with a comprehensive evaluation of the patient's general health
• Administers dental anesthesia to minimize pain during procedures
• Repairs decayed and broken teeth and performs extractions when necessary
• Refers patients to dental specialists and ensures appropriate follow-up of the care plan
• Adheres to HIPAA and OSHA regulations and other applicable federal/state rules
