Job summary

Your Health Organization is seeking a Registered Nurse to join our urgent care team, delivering compassionate patient care across homes, facilities, clinics, and virtual visits. You will coordinate care with providers, support CCM plans, and participate in telehealth and hospice coordination to ensure quality outcomes.

Responsibilities include admission, care coordination, EMR documentation, and collaboration with a multidisciplinary team.

Qualifications

• Must be a licensed nurse in good standing.
• Minimum of three (3) years clinical experience preferred.
• Demonstrated ability to supervise and direct professional and administrative personnel.
• Ability to read and communicate effectively.
• Ability to manage and demonstrate effective leadership skills.
• Strong interpersonal and communication skills under all conditions.

Responsibilities

• Admission and triage of patients in various care settings.
• Coordinate care through communication with providers and care teams.
• Collaborate to revise CCM plan of care as needed.
• Facilitate telehealth visits with nurse practitioners for coordination of care.
• Coordinate with hospice interdisciplinary team and care plan participants.
• Participate in hospice IDG meetings and care coordination activities.
• Collaborate with physicians, nurses, therapists, home health or hospice staff.
• Document observations and ensure accuracy in EMR.
• Communicate with patients, families, and caregivers; follow up on care plans.
• Support quality improvement initiatives and comply with organizational policies.

Health Care Professionals will service homes and/or Assisted Living Facility patients in care coordination with the provider within their assigned service area(s). This role would work closely with our specialist department to assist in the different settings.

What Are We Looking For?

Your Health is currently looking for a Registered Nurse to join our growing urgent care family.

The following is a list of essential functions, which may be subject to change at any time and without notice. Management may assign new duties, reassign existing duties, and/or eliminate function(s).

• Admission
• Communication, collaboration, and coordination of care
• Continuous review and revision of CCM plan of care
• Facilitate a telehealth visit with the nurse practitioner for coordination of care.
• Coordinate with the patient hospice interdisciplinary team and other individuals in the patient's care plan.
• Participate in the hospice IDG meeting
• Assisted Living & Skilled Nursing Facilities
• Finding with physicians in assisted living or skilled nursing facilities
• Coordinate with the patient health care team, physicians, physical and occupational therapists, home health or hospice representatives and other individuals in the patient's care plan.
• Assess and chart observations of the patient condition.
• Collaborate with nurse practitioners to satisfy each patient quality measures.
• Facilitate telehealth or tele-pharmacy visits
• Ensure follow-up visits are scheduled
• Participate in quality improvement projects i.e. root cause analysis of adverse incidents
• Maintain current list of providers and specialists updated in the EMR
• Communicate with patients, families, and caregivers.
• Utilize the company software systems and update information as required.
• Participate in coaching calls.
• Perform other duties as requested or required, in the sole discretion of SC House Calls.

QUALIFICATIONS

Must be a licensed nurse. License must be in good standing with the appropriate board.
