Job summary

You will see patients of all ages in-person, order tests, and manage chronic conditions while educating on lifestyle and preventive strategies.

Qualifications

• Must hold current unrestricted NP license and OH eligibility.
• Experience delivering primary care across lifespan, including prevention.
• Interest or experience in functional/integrative medicine and root-cause approaches.
• Strong clinical assessment, diagnostic, and care plan skills.
• Clear, empathetic communication fostering trust with patients and families.
• Comfort with membership-based care models and longer visits.

Responsibilities

• Provide primary care to patients of all ages in a functional medicine setting.
