Job summary

Ascension at Home together with Compassus is seeking a Home Health Registered Nurse (RN) for a Baylor shift (Friday–Sunday) in Jacksonville, FL. This fully benefited role supports patient recovery at home, educates families on disease management, and provides clinical oversight to an interdisciplinary team.

The position requires an RN license in the state, 2 years of RN experience (home health preferred), a valid driver’s license, and CPR/BCLS certification.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, home health experience strongly preferred.
• Current driver's license.
• Current CPR and BCLS certification.

Responsibilities

• Support patient recovery and ability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy.
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Case manage and provide clinical oversight to interdisciplinary team.
