Position Summary

We are currently seeking a Full-Time Physical Therapist (PT/RPT) to join our home care team. This role provides consistent full-time hours (40 hours per week), competitive compensation, comprehensive benefits, performance-based bonuses, and the opportunity to make a meaningful impact by delivering one-on-one care in patients’ homes.

Key Responsibilities

• Provide comprehensive and compassionate physical therapy services in patients’ homes in accordance with physician orders.
• Perform initial evaluations and ongoing assessments of patients’ functional abilities.
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and quality of life.
• Deliver skilled therapeutic interventions, exercises, and patient/caregiver education.
• Document patient progress accurately and in a timely manner per company and regulatory standards.
• Collaborate with nurses, caregivers, physicians, and interdisciplinary teams to ensure coordinated care.
• Maintain a safe, supportive, and professional therapy environment at all times.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT/RPT) license in Massachusetts.
• Prior experience in home care of 2 years, skilled nursing, or sub-acute rehab preferred but not required.
• Strong communication, interpersonal, and clinical documentation skills.
• Reliable transportation and ability to travel within assigned service areas.
