Job summary

Agrace is seeking a Spiritual and Grief Counselor in Janesville to join our interdisciplinary team. The role involves assessing patients and families, developing individualized care and bereavement plans, and providing both individual and group spiritual support.

Qualifications

• Degree in Social Work, Counseling (or related) or equivalent education/experience
• At least one CPE unit (required unless Social Work degree)
• Two+ years of counseling experience preferred
• Valid driver's license, auto insurance, and reliable vehicle
• Experience providing counseling/spiritual care in group and individual settings
• Experience handling grief related to death
• Intermediate Microsoft Office skills
• Ability to communicate effectively verbally and in writing
• Ability to maintain confidentiality
• Ability to work at multiple Agrace locations
• Ability to adapt time/tasks for organizational needs
• Awareness of body mechanics and standard precautions

Responsibilities

• Respond to referrals to establish counseling relationships and expectations
• Perform spiritual and grief assessments, interventions, and care planning
• Coordinate care plans with interdisciplinary team per guidelines
• Collaborate with patients' faith communities and refer to resources as needed
• Develop individualized bereavement plans and provide updates to the team
• Lead community grief groups and memorials
• Provide supervision/support to counseling volunteers
• Participate in internal work groups and committees
• Provide counseling support to teams and community partners
