LoveWell Hospice is seeking a dedicated RN Case Manager to serve patients and families throughout the Frisco/Lewisville area from our Allen, TX base. The role focuses on coordinating clinical care, guiding families, and delivering compassionate end-of-life hospice services in both community settings and private homes.

The ideal candidate has Texas RN licensure, hospice experience preferred, and excellent communication and documentation skills.

Qualifications

• Active Texas RN license.
• 2 years of Hospice agency experience preferred.
• FireNote experience preferred.
• Good communication skills, assessment skills, and strong clinical documentation skills.
• Ability to read, write, and speak English clearly.

Join our LoveWell team as a Full-Time RN Case Manager serving patients and families throughout the Frisco/Lewisville area. In this role, you'll provide expert clinical care, compassionate support, and guidance to patients and their loved ones during some of life's most meaningful moments. You will serve patients in both community settings and private homes, building meaningful relationships while delivering exceptional hospice care. This is an exciting opportunity to join a dynamic team and help grow our presence throughout the territory. While you'll spend most of your time supporting patients in the field, you'll only be required to report to our Allen office approximately three times per month.
