Job summary

Care Pavilion Nursing & Rehabilitation Center in Philadelphia, PA is seeking a full-time Physical Therapist to join our in-house rehab team. PRN opportunities are also available for flexible scheduling.

You will evaluate residents, develop individualized plans of care, provide skilled PT services, and collaborate with nursing and therapy staff to improve mobility, safety and quality of life for our SNF/LTC residents.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapist license or eligibility for licensure.
• Strong communication and clinical skills.
• Ability to work effectively as part of an interdisciplinary team.
• SNF/LTC experience is a plus.
• Experience with Net Health and PointClickCare (PCC) is a plus.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy services to SNF/LTC residents.
• Help residents improve strength, mobility, balance, safety, and functional independence.
• Establish measurable functional goals and monitor resident progress.
• Complete accurate and timely documentation.
• Collaborate with interdisciplinary team members to support positive resident outcomes.
• Provide resident, family, and caregiver education as appropriate.
