Job summary

Qualifications

• Active PT license in the state of practice.
• Graduate of CAPTE-accredited program; if foreign-trained, program must be deemed substantially equivalent by a credential evaluation organization approved by APTA.

Responsibilities

• Perform evaluations, assessments, and treatments per PT care plan.
• Complete documentation in a timely and accurate manner.
• Follow policies and procedures related to scope of practice.
