Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care, developing appropriate care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Role involves standing, walking, lifting, transferring, and repositioning patients and equipment.
• Administer procedures including manual techniques, ambulation, exercises, modalities, and wound care.
• Provide counseling to patients, families, and other providers about physical disorders.
• Generate discharge summaries compliant with regulatory and clinical requirements.
