Job summary

Functional Pathways in Evanston, IL, seeks a full-time Physical Therapist to screen, evaluate, re-evaluate, and discharge plans under physician supervision. Westminster Place is the care setting and community environment.

Requires degree in physical therapy, active state license, strong organization and communication skills, ability to supervise PTAs, and knowledge of Functional Pathways policies. A $5,000 sign-on bonus is offered to direct applicants.

Qualifications

• Bachelor’s, Master’s or Doctorate in physical therapy from an accredited program.
• Active state license required.
• Excellent organizational and time management skills.
• Excellent written and verbal communication skills.
• Possess knowledge of Functional Pathways policies and procedures.
• Ability to supervise Physical Therapy Assistants.

Responsibilities

• Screen, evaluate, and re-evaluate patients.
• Discharge planning and revisions to plans of care as supervised by the physician.
• Maintain adherence to clinical programs and procedures.
