BAYADA Home Health Care is seeking an Occupational Therapist to join our Oahu Visits team in Honolulu. You’ll evaluate clients in their homes, craft personalized treatment plans, and collaborate with a multidisciplinary team to support independence and safe living at home.

Qualifications

• Perform thorough evaluations of patients’ functional status.
• Develop individualized intervention plans to achieve goals.
• Assess home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations for better outcomes.
• Educate patients and families on rehabilitative care for health and safety.
• Document observations, interventions, and evaluations using a touch pad tablet.
