Job summary

Responsibilities

• Performs patient evaluations by establishing an occupational profile and goals.
• Develops an individualized plan of care with patient-centered interventions.
• Conducts ongoing reassessment and monitors progress.
• Initiates and manages discharge planning from first visit onward.
• Documents care clearly within policy and regulatory standards.
• Communicates with patient and care team to promote engagement and safety.
• Provides patient/family education on therapies and home program.
• Participates in quality improvement activities.
