Job summary

LiveWell Homecare Agency, LLC is seeking a registered nurse to provide independent, skilled patient care in a clinical/homecare setting. The RN will assess, plan, intervene, and evaluate patient needs while coordinating with care providers for quality outcomes.

The role requires strong documentation, medication administration, and education for patients and families. The night shift from 7 PM to 7 AM offers an opportunity to support vulnerable patients with compassionate, evidence-based care.

Qualifications

• RN license required and BLS certification; ACLS required.
• Bachelor of Science in Nursing preferred.
• Completion of onboarding and annual competency requirements.
• Additional certifications such as PALS, TNCC, NRP, and Fetal Heart Monitoring may be required within timeframes.

Responsibilities

• Utilize the nursing process for assigned patients (assessment, diagnosis, planning, intervention, evaluation).
• Address psychological, emotional, cultural, and social needs of patients and families per practice standards.
• Administer medications and treatments, monitor side effects and effectiveness.
• Document patient history, symptoms, medications, and care provided.
• Educate patients, families, and caregivers, identifying issues and resources.
