Job summary

Lifespace Communities, headquartered in Dallas, Texas, is seeking a Full Time Licensed Practical Nurse for the 10p to 6a shift at the Barton Creek location. You will participate with the interdisciplinary team to review, plan, coordinate and evaluate residents’ care, document nursing needs, and initiate physician orders.

The role requires an LPN graduate with an active license; you will manage medications, communicate with families, coordinate admissions and transfers, and ensure care plans

Qualifications

• Must be a Licensed Practical Nurse with an active state license.
• Experience coordinating with interdisciplinary teams preferred.
• Ability to document care accurately and timely.

Responsibilities

• Review, plan, coordinate, and evaluate resident care with the team.
• Document resident condition and nursing needs accurately.
• Administer and chart medications; initiate physician orders.
• Coordinate admissions, discharges, and transfers.
• Communicate changes in medications with families.
• Ensure care plans reflect resident individuality.
