Zusammenfassung

ChristianaCare HomeHealth is seeking a Full Time Medical Social Worker to support Dover, Smyrna, Frederica and surrounding areas. Workdays are 8:30am-5pm with occasional weekends or holidays based on department needs.

You will participate in care conferences, provide social work evaluations, and coordinate with the care team to plan patient transitions home. Benefits include tuition assistance, medical/dental/vision coverage from day one, and retirement options.

Qualifikationen

• Master’s degree from a school of social work accredited by the council on Social Work Education.
• 2 or more years experience in health care setting required.
• Prefer 1 year of home care or community field experience.

Aufgaben

• Participates in Case Conferences and develops total plans of care with the multidisciplinary team.
• Provides clinical social evaluations and plans appropriate interventions based on findings.
• Identifies patients appropriate for specialty programs and Telehealth.
• Conducts home visits and discusses evaluation findings with Senior Medical Social Worker.
• Identifies community resource referrals to address client and family needs.
