Job summary

First Choice Health Centers in East Hartford, CT is seeking an experienced Licensed Practical Nurse to provide direct patient care and coordinate urgent calls across disciplines. The LPN will support medication refills, schedule coordination, and patient counseling in a fast-paced clinical setting.

The position requires a Connecticut LPN license and BLS certification, with competitive hourly pay and benefits.

Qualifications

• A valid LPN license in the state of Connecticut.
• BLS Certification.
• Experience with an electronic health record system.
• Excellent communication and interpersonal skills.
• A positive attitude and willingness to be cross-trained in multiple specialty areas, including pediatrics.

Responsibilities

• Provide direct and individualized nursing care to patients based on the nursing process.
• Maintain accurate medication inventory listings and oversee CT Vaccine Program documentation.
• Document in the electronic health record all assessments, vital signs, specimens, interventions and responses.
• Manage prescriptions and medication refills for providers; perform wound care with proper infection control.
• Complete meaningful use documentation and refer patients to specialty providers as needed.
• Collaborate with medical assistants to ensure safe, effective patient care and teamwork.
• Monitor telephone encounters and follow up on overdue orders and critical referrals.
• Adhere to patient safety standards and infection control guidelines.
