Job summary

River Valley Home Health and Hospice is seeking a Full Time Hospice Social Worker to implement standards of care for medical social work services and ensure adherence to service agreements.

The role involves psychosocial assessments, in-home evaluations, counseling patients and families, and coordinating with the interdisciplinary team to support end-of-life care. Strong communication, organization, and CPR certification are essential.

Qualifications

• Bachelor's in social work accredited by the Council on Social Work Education (CSWE).
• Minimum of one year’s experience in health care; hospice experience preferred.
• Understanding of hospice philosophy, and death/dying issues.
• Experience in hospice care is preferred.
• Demonstrates good verbal and written communication and organizational skills.
• Possesses and maintains current CPR certification.
• Must have valid driver’s license and access to automobile/transportation.

Responsibilities

• Assess psychosocial status of patients and families/caregivers and communicate findings to the care team.
• Conduct in-home assessments and plan alternative arrangements when safety concerns exist.
• Perform social evaluations and plan interventions based on findings.
• Counsel patients and families about stress and coping.
• Maintain clinical records for patients referred to social work.
• Provide information and referrals for patients and families regarding practical and environmental needs.
• Liaise between patients, families, and community agencies.
• Maintain collaborative relationships with organization personnel to support patient care.
• Develop and manage contracts with public and private agencies as resources.
• Participate in development of the individualized plan of care and attend interdisciplinary meetings.
• Assist clinicians in understanding social and emotional factors related to health problems and death.
• Participate in quality improvement activities.
• Perform other duties as delegated by the Clinical Supervisor.

JOB SUMMARY

The medical social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.

DUTIES & RESPONSIBILITIES

• Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
• Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
• Carries out social evaluations and plans intervention based on evaluation findings.
• Counsel’s patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
• Provides information to patients and families/caregivers and community agencies.
• Serves as liaison between patients and families/caregivers and community agencies.
• Maintains collaborative relationships with organization personnel to support patient care.
• Maintains and develops contracts with public and private agencies as resources for patient and personnel.
• Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings.
• Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
• Actively participates in quality assessment performance improvement teams and activities.
• Other duties as delegated by the Clinical Supervisor.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

• A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).
• Minimum of one year's experience in health care, hospice experience preferred.
• Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains current CPR Certification.
• The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.
• Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.
