LeoForce in New York City seeks a mid-level Physician Assistant to provide palliative care consults across hospitals, SNFs, and community settings in Brooklyn and Manhattan. You will collaborate with physicians, NPs, and IDT to assess and plan hospice eligibility and symptom management.

You will self-schedule visits, perform comprehensive assessments, participate in on-call rotations, and contribute to quality improvement efforts to support high-quality palliative care delivery.

Qualifications

• Active Physician Assistant license in New York State in good standing.
• Master of Science in Physician Assistant Studies from an accredited program required.
• 1–2 years clinical experience as a licensed PA preferred, with palliative care, hospice, geriatrics, or related fields.
• Current DEA registration allowing prescription of controlled substances.
• CAQ in Hospice and Palliative Medicine preferred.
• Proficiency in Microsoft Word, Excel, and electronic health records (EHR).
• CPR/BLS required.
• Valid New York State driver’s license preferred.

Responsibilities

• Provide palliative care consults with pain and symptom management and hospice eligibility evaluations.

Job Details

Responsibilities:

• The Physician Assistant Provider will work in a culturally diverse environment with patients and families of any background and will be collaborating with Physicians, Nurse Practitioners, and IDT in providing palliative care consults with patients regarding pain and symptom management, advance care planning, and hospice eligibility evaluations. Initial patient encounters are performed in response to consultation requests, with ongoing visits as appropriate and Provider must be proficient in comprehensive assessment and management in advanced illness and be knowledgeable in hospice eligibility criteria and recertification processes.
• The Physician Assistant will work in various settings, including hospitals, skilled nursing facilities, and the community, covering (2) Boroughs: Northern Brooklyn, and the Lower Half of Manhattan.
• PA – C will deliver patient-centered, goal-directed care for individuals with serious illness.
• The PA will self-schedule all patient visits and will independently conduct comprehensive patient assessments, treatment planning, and follow-up care, ensuring care plans minimize illness burden and align with patients’ goals and values.
• The role includes participating in on-call rotation, documentation, interdisciplinary collaboration, education, and quality improvement initiatives to support high-quality palliative care delivery.

Qualifications:

• Active Physician Assistant license in New York State in good standing with NO restrictions
• Master of Science in Physician Assistant Studies from an accredited program required.
• Minimum 1–2 years clinical experience as a licensed Physician Assistant preferred, with a focus in palliative care, hospice, geriatrics, or related fields.
• Experience in skilled nursing facilities or community-based care is preferred.
• Current DEA registration allowing prescription of controlled substances.
• Certificate of Added Qualifications (CAQ) in Hospice and Palliative Medicine preferred.
• Proficiency in Microsoft Word, Excel, and electronic health records (EHR).
• CPR/BLS required.
• Valid New York State driver’s license preferred.
