Job summary

Enliven is seeking an experienced Registered Nurse (RN) to service home infusion patients in Fort Worth, TX and surrounding locations. The ideal candidate will educate patients and families on prevention and healthy habits and provide high-quality nursing care with little supervision.

The role requires a strong clinical background with Med/Surg/ICU/ER experience, BLS/CPR, and the ability to handle emergent situations. Home health/infusion experience is preferred.

Qualifications

• Minimum of five years nursing experience preferred in acute care setting.
• Strong Med/Surg, ICU, ER, acute experience.
• Current CPR/BLS required.
• ACLS/PALS preferred but not required.

Responsibilities

• Complete daily EMR forms for each patient.
• Provide admission, case management, and follow-up skilled visits at home and infusion centers.
• Administer ongoing care/infusions and follow physician orders and policies.
• Develop initial treatment plans with physicians based on assessments.
• Discharge patients after physician/manager consultation and complete documentation.
• Prepare and update patient records and care plans; report progress to physicians and managers.

Job description

Enliven is seeking an experienced Registered Nurse (RN) to service home infusion patients in Fort Worth, TX,and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care.

Responsibilities:

• Complete daily electronic forms for each assigned patient within a secured Electronic Medical Record (EMR) System.

• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.

• Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.

• Confer with physicians and pharmacies in developing the initial plan of treatment based on the physician's orders and initial patient assessment.

• Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.

• Report patient care/conditions/progress to the patient's physician/pharmacy and Clinical Manager continuously.

• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.

• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.

• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.

• Prepare visit/shift reports, update/summarize patient records, and confer with other health care disciplines to provide optimum patient care.

Schedule

• Monday - Friday

• On-call weekend rotations every 6 to 8 weeks

Job Requirements:

• Background check required

• Malpractice insurance required

• Drug screen required

• Associate or Bachelor Degree in Nursing Minimum of five years of nursing experience preferred in an Acute Care setting

• Strong Med/Surg, ICU, ER, acute experience

• Home Health/Infusion experience (not required, but preferred)

• Current CPR/Basic Life Support certifications (BLS) required

• Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS) (not required, but preferred)

• Skilled in accessing Implanted Ports; Central Lines; PICC lines

• Strong IV placement skills

• Strong organizational and communication skills
