Job summary

Archbold Home Health in Bainbridge, GA, part of the LHC Group and Optum family, seeks an experienced Registered Nurse to provide and direct home-based nursing care. You will coordinate care with physicians, agency staff, patients, and families to maintain continuity of care and patient safety across Grady, Decatur, Seminole, and Mitchell counties.

Responsibilities include planning and evaluating care, supervising home health staff, completing OASIS assessments, and ensuring regulatory

Qualifications

Current and unrestricted RN licensure in state of practice.
Current Driver's License with access to a dependable vehicle or transportation.
CPR certification or ability to obtain within 90 days.

Responsibilities

Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
Supervises care provided by home health aides and licensed personnel; provides instruction and assigns tasks.
Provides required supervisory visits and ensures compliant documentation.
Completes comprehensive assessments (OASIS) and medication reconciliation accurately.
Documents patient visits per policy and payer requirements; coordinates with care team.
Communicates with physicians, patients, and family to ensure quality care and service excellence.
Participates in on-call and weekend rotation as needed and supports survey readiness.
