Job summary

AdvisaCare in Grand Rapids, MI is seeking a Registered Nurse to provide in-home skilled nursing care, assessments, and patient education.

You will follow personalized care plans, administer medications, collaborate with a multidisciplinary team, and ensure safety and documentation standards.

Qualifications

• Active Registered Nurse license in the state of practice.
• Minimum of 1 year home health nursing experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Valid driver's license and reliable transportation.
• Ability to work independently and manage caseload effectively.
• CPR certification.
• Pass drug screening and background checks as required.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
