Job summary

Aveanna Healthcare is seeking a Physical Therapy Assistant for a full-time home health role serving Bridgeville, Seaford and Delmar. You will perform skilled PT services, educate families, and coordinate with the care team to support patient independence and recovery.

Requirements include PTA degree, active PTA license, and CPR. This position emphasizes compassionate care, adherence to the plan of care, and timely documentation across visits.

Qualifications

• Graduation from a Physical Therapist Assistant program from an accredited institution.
• Current, unrestricted state license as a PTA
• Valid CPR certification.

Responsibilities

• Provides direct physical therapy per the supervision and plan of care.
• Communicates plan of care updates to patient and caregiver.
• Delivers skilled therapy and procedures as appropriate for patient care and safety.
• Reports patient observations to supervisor and physician as appropriate.
• Reinforces safety and rehabilitation goals with patients and families.
• Educates patients and caregivers on devices and rehabilitation techniques.
• Collaborates with other disciplines on the care plan.
• Prepares discharge instructions and communicates discharge plan to family.
