Job summary

Excelin Home Health in Yuba City, CA is seeking a full-time Physical Therapist to provide therapy services under a physician's plan of care, evaluate patients, develop and adjust the total plan of care, and supervise therapy assistants as required.

Qualifications

• Requires degree in physical therapy approved by an accredited organization.
• Licensed to practice as a physical therapist in the state where employed.
• Two years of experience; home health/community care preferred.

Responsibilities

• Provide physical therapy services per physician's plan of care.
• Assess and develop the total plan of care for patients.
• Instruct patients and families in use and care of therapeutic devices.
• Direct PT treatment and monitor patient progress.
• Supervise PT assistants and home health aides as needed.
• Prepare and submit clinical progress summaries and reports.
