Job summary

Care Navigators On Demand is seeking a Case Manager to coordinate patient care within a hospital setting, ensuring efficient use of resources and adherence to care plans. You will support patients through assessment, planning and discharge processes, influencing outcomes and satisfaction.

As aCase Manager / CMyou will:

Requirements

• Appropriateminimum degree for preferred licensure or certification or commensurateexperience in a related healthcare field.
  
• Healthcareprofessional licensure required as Registered Nurse, Licensed ClinicalSocial Worker (LCSW) or Licensed Social Worker (LSW).
  
• Appropriatecertification in Case Management preferred; for example, Commission forCase Manager Certification (CCMC); Association of Rehabilitation Nurses(ARN) certification.
  
• Fiveyears of experience in an acute care Medical/Surgical and/or ICU/CCU setting.
