FOX Rehabilitation is seeking a licensed Physical Therapist to deliver in-home and senior-living care to older adults across Jefferson, WI, with some telehealth component. You will work autonomously on a schedule that fits patient needs, building lasting patient relationships while documenting care in our EMR and coordinating with an interdisciplinary team.

Qualificações

• Current physical therapy license in the state(s) of practice.
• DPT degree from an accredited program.
• Ability to manage your own treatment schedule across settings (home, senior living, telehealth).

Responsabilidades

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Deliver therapy sessions to improve mobility, independence, and quality of life.
• Document thoroughly in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
