Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one geriatric care in patients' homes or senior living communities in Hanahan, SC. You will build a flexible schedule and help older adults improve mobility, independence, and quality of life through our Geriatric House Calls model.

Telehealth and other virtual care options may be incorporated to extend access, with collaboration from an interdisciplinary team and thorough documentation in FOX’s EMR system to meet

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings
• Deliver therapy sessions that support mobility, independence and functional goals
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Provide telehealth/virtual care when appropriate to extend access and flexibility
