Responsibilities

• Perform daily tracheostomy care, suctioning, stoma assessments, and tube changes as per protocol.
• Monitor ventilator settings and alarms and respond to respiratory needs.
• Administer medications and treatments as ordered by physician.
• Provide informed care respecting individual communication preferences.
• Educate Direct Support Professionals on care protocols and safety measures.
• Document nursing assessments, medications, and treatments per ICF regulations.
• Collaborate with interdisciplinary team including physicians and therapists.
• Support respiratory weaning and decannulation when ordered.

Essential Job Functions:

• Deliver skilled nursing care to ICF residents with tracheostomies and or mechanical ventilation needs in accordance with physician orders and regulatory compliance.
• Perform daily tracheostomy care, suctioning, stoma assessments, and tube changes as per protocol.
• Monitor ventilator settings and alarms and promptly respond to respiratory needs.
• Administer Medication and Treatments as ordered by physician in accordance with the Medication Administration Policy.
• Provide informed care that respects the individuals preferred communication style.
• Provide support and education to Direct Support Professionals and other team members on care protocols, safety measures, and emergency protocols.
• Document nursing assessments, medications, treatments, in electronic and paper form according to ICF regulations.
• Collaborate with interdisciplinary team members including physicians, respiratory therapists, behavioral support, speech, occupational, and physical therapies, direct care, quality and residential management.
• Provide support with respiratory weaning and decannulation processes when ordered.

Requirements

Qualifications:

• Licensed RN or LPN in Pennsylvania
