Job summary

NHC is seeking a dedicated Dietitian to assess nutritional needs, develop, implement, and monitor care plans for patients in our long‑term care setting. You will ensure compliance with federal, state, and center guidelines, and collaborate with an interdisciplinary team to optimize patient outcomes.

Ideal candidates hold a Bachelor's or Master's in Nutrition or Dietetics, with RD credentials (RDN) and current licensure.

Qualifications

• Strong organizational, communication, clinical, and interpersonal skills.
• Knowledge of long‑term care regulations and nutritional standards.
• Basic computer and documentation skills.

Responsibilities

• Complete and document admission, quarterly, annual, significant-change, and high-risk patient nutrition assessments.
• Develop and monitor individualized nutrition care plans.
• Provide nutrition education and training to food service and center staff.
• Collaborate with nursing, therapy, administration, and food and nutrition services to promote optimal patient outcomes.
• Participate in care plan meetings and interdisciplinary committees.
• Lead Quality Assurance and Performance Improvement (QAPI) initiatives, conduct audits, and implement action plans for continuous improvement.
• Complete MDS assessments and documentation.

Position Summary

• Basic computer and documentation skills

Schedule

In‑person, Monday–Friday (80 hours bi‑weekly), with flexibility within the week.
