Qualifications

• Licensed Vocational Nurse (LVN) in the state of Texas.
• Minimum of 1 year of home health experience preferred.
• Strong knowledge of patient care, infection control, and documentation practices.
• Ability to follow and implement a Plan of Care (POC) under RN supervision.
• Strong clinical assessment, documentation, and communication skills.

Responsibilities

• Provide skilled nursing care to patients in the home setting under the supervision of a Registered Nurse.
• Assist in identifying patient physical, psychosocial, and environmental needs.
• Participate in developing and implementing the Plan of Care (POC) in collaboration with the RN.
• Administer treatments and interventions in accordance with physician orders and agency policies.
• Monitor patient condition and report changes to the supervising RN.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
• Participate in interdisciplinary communication and weekly case conferences (IDG/IDT).
• Provide verbal and written updates to the RN prior to reassessment, supervisory, and discharge visits.

Job Summary

TheraCare Home Health is seeking a dependable and patient-focused Licensed Vocational Nurse (LVN) to provide skilled nursing care in the home setting under the supervision of a Registered Nurse. In this role, you will support patient care delivery, assist with care planning, and collaborate with the interdisciplinary team to ensure high-quality outcomes. Qualifications

• Licensed Vocational Nurse (LVN) in the state of Texas
• Minimum of 1 year of home health experience preferred
• Strong knowledge of patient care, infection control, and documentation practices
• Ability to follow and implement a Plan of Care (POC) under RN supervision
• Strong clinical assessment, documentation, and communication skills
• Ability to identify and report changes in patient condition
• Experience working with interdisciplinary teams in a home health setting preferred
• Reliable transportation with valid driver’s license and current auto liability insurance
• Ability to read, write, and communicate effectively in English

Essential Functions
