Provider Partners Health Plan is seeking a Nurse Practitioner to oversee coordinated care for PPHP members in designated skilled nursing facilities. You will collaborate with primary care physicians to develop and implement an individual plan of care.
You will serve as medical liaison to the interdisciplinary team, perform preventive care, diagnose acute and chronic conditions, order tests, prescribe medications, and guide transitions of care in accordance with state licensure and PPHP standards.
The Institutional Special Needs Plan (ISNP) Nurse Practitioner is responsible for coordinated and comprehensive care of Provider Partner Health Plan (PPHP) members at designated skilled nursing facilities. The Nurse Practitioner works in collaboration with the member’s primary care physician to deliver exception health care. The nurse practitioner is the medical liaison to the interdisciplinary staff and allied health providers and communicates effectively to assure seamless delivery of services. As a key member of the clinical team in the skilled nursing facility, the Nurse Practitioner provides preventive care, diagnoses both acute and chronic illnesses, orders and interprets diagnostic tests, prescribes medications, refers to specialists as appropriate, establishes a plan of care with short-term and long-term goals, and is a resource to members, families, and staff.
