Job summary

Elevance Health is seeking a Foster Care Engagement Specialist in North Carolina within 50 miles of Rowan, Stanly, or Mecklenburg counties. The role supports care coordination teams, connects members and caregivers to resources, and helps resolve challenges with local agencies.

The position requires a high school diploma or equivalent and at least 2 years of related experience; strong interpersonal skills and teamwork are essential.

Qualifications

• Requires a HS diploma or equivalent, or any combination of education and experience providing an equivalent background.
• Interpersonal skills and ability to work well with a team are essential; computer literacy is expected.

Responsibilities

• Identify opportunities for engagement of members and their caregivers and families to share with care management team.
• Partner with community peer support services to create referral pathways and link members to appropriate services.
• Support care managers as they advocate for foster parents and caregivers with various agencies and systems.
• Develop strategy for peer support programs.
• Conduct face-to-face wellness assessments.
• Facilitate access to community resources.
