Job Summary

The Foster Care Case Manager is responsible for performing duties related to the Foster Care Ministry, enabling foster families and children in care to operate at the highest level of competency in carrying out the ministry’s and Agency’s policies, procedures and programs. Responsibilities include teamwork, group work, working with families according to the developed plan, family and custodian contacts, community resources, and case record management.

Qualifications
  • Minimum of 23 years of age.
  • Bachelor’s Degree in Human Services or related field, from an accredited university or accredited seminary.
  • Two years of experience with the population served.
  • Valid driver's license and ability to operate an agency vehicle to transport children.
  • Capability to manage, plan, organize, implement, monitor and maintain agency programs and adhere to accreditation standards and N.C. licensing requirements.
  • Meets the general qualifications for employment as outlined in the institution’s personnel policies.
  • Respects the cultural diversity of children and strives to create a positive relationship.
Essential Duties and Responsibilities
  • Prepare for and participate in regular supervisory conferences.
  • Participate in 90‑day and annual staff performance evaluations.
  • Keep supervisor informed on the current status of total work responsibilities.

  • Function within the guidelines of the job description and the Program of Intentionalized Services.
  • Represent the agency’s philosophy, purpose and programs to groups, individuals and referral agencies in a professional and objective manner.
Professionalism and Teamwork
  • Demonstrate knowledge of and ability to work as a member of the foster home team.
  • Have knowledge of and support the roles of other team members.
  • Respect the right of the team to be involved with the child and his/her family.
  • Share with team members information necessary for foster home and treatment team decision.
Team Collaboration
  • Support the team in providing daily care to meet the needs of children and maintain a positive environment.
  • Participate as a team member in team meetings and other staff meetings.
  • Participate in ongoing monitoring of effectiveness of the team’s functioning.
  • Research and conduct foster parent training on appropriate subjects as assigned.
  • Provide leadership in creating and maintaining therapeutic milieu.
  • Work closely with the Division of Social Services Social Worker.
  • Respect the cultural and socioeconomic diversity of staff members and children in care and strive to create a positive relationship within BCH.
  • Participate in the agency’s Performance Quality Improvement (PQI) program as needed.
Clients and Foster Families
  • Facilitate FAC, admission, intake, transfer and discharge conferences.
  • Provide leadership in child and family conferences; coordinate and implement the Plan of Service and Plan of Service Reviews with each family.
  • Recruit and license foster families; provide and/or facilities required training.
  • Provide support, encouragement, recognition of feelings and problem‑solving which compliments family work and family therapy.
  • Arrange family contacts, emergency contacts, financial accounting, scheduling of appointments, behavior infractions, obtaining consents, in‑home plans and reporting medical, school and other daily events.
  • Establish and maintain relationships with resources for client systems and assist family/custodian in securing appropriate internal and external community resources to meet the family’s needs.
  • Conduct psycho‑educational groups covering peer pressure, social skills, anger management, independent living, group building initiatives, communication, decision making, chemical dependency and self‑esteem.
  • Support medical, dental and other appointments for youth in care.
  • Set the tone that clarifies BCH Family Systems approach to family work, if the goal is family reunification.
  • Conduct routine home visits to the foster home.
  • Recruit foster families by presenting the need for foster homes to individuals and groups.
  • Conduct foster home application studies in keeping with BCH policies, procedures and licensing standards.
  • Submit all required paperwork for foster home initial licensing and renewals.
  • Orient and train new foster parents to understand their roles, responsibilities and expectations.
  • Provide ongoing training to meet state and accreditation requirements and special needs of foster parents.
  • Be available to foster parents on a regularly‑scheduled and emergency basis to assist with identified issues and crisis needs.
  • Assist foster parents with securing community resources identified as needed by the team.
Record Keeping
  • Maintain all client files and documentation of case activities current, including admission documents, MIGS, Plan of Service, Plan of Service Review, summary dictations, individual crisis management plans, outcome measurements, consents, progress notes, admission, transfer, discharge memos, alerting slips, court reports and aftercare plans.
Knowledge and Training
  • Continuously increase knowledge and skills by staying current with trends in the field of foster care.
  • Consistently apply professional knowledge and skills in fulfilling the responsibilities of the position.
  • Possess the knowledge and skills necessary to carry out the responsibilities of the position.
  • Demonstrate a current knowledge of trends in services to families and children.
  • Develop a pool of external resources for supporting services as needed.
  • Demonstrate and implement a working knowledge of skills in working with family systems, individuals and groups.
  • Participate in Family Systems Trainings.
Faith and Mission Alignment (BCH Requirement)
  • Profess to be a Christian believer.
  • Submit an individual Statement of Faith, Acknowledgement and Pledge as part of employment requirements.
Equipment

Ability to operate standard office machines such as copier, computer, fax, calculator, etc. Ability to work within Microsoft Outlook, Excel and Word software programs.

Language Skills

Ability to read, write and complete necessary documentation. Ability to effectively present information and respond to questions from clients, customers, resources and the general public.

Mental, Visual and Auditory Demand

Ability to interpret a variety of instructions in written, oral, diagram or schedule form and draw conclusions.

Regular contact with colleagues in all departments and with outside individuals and groups; should represent Baptist Children’s Homes of NC, Inc. in a positive manner. External contacts require appropriate handling of situations to protect the agency’s image.
